U.S.A Rebecca R. Hill, PhD, DNP, RN FNP-C

Rebecca Russo Hill, PhD, DNP, RN, FNP-C earned her BSN from the University of Rhode Island in 2006, her MSN and post-master’s certificate in nursing education from Duke University in 2010, her DNP from Duke University in 2013 and her PhD in Nursing from Boston College in 2021. Rebecca is a family nurse practitioner and certified as a lactation counselor and nurse educator. She is the current program director of prelicensure nursing programs at the MGH Institute of Health Professions in Boston, MA. Rebecca’s dissertation research focused on symptoms of problematic feeding in infants with tongue-tie. She will collaborate with other health profession disciplines to advance the state of the science surrounding infant feeding, tongue-tie, and frenotomy. Her dissertation research has provided the foundation for a lifelong program of research to improve health care for mothers and their children. She is currently studying the effects of frenotomy on non-nutritive sucking mechanics.

Rebecca is an active member of several professional organizations including the American Nurses Association (ANA), American Association of Nurse Practitioners (AANP), the National League for Nursing (NLN) and Sigma Theta Tau International (STTI). Rebecca resides in Massachusetts, raising three children alongside her husband Jonathan.

Abstract:

Despite the low-level of evidence supporting the correction of tongue-tie for breastfeeding problems, recognition and treatment has increased substantially over the last 20 years. Recent systematic reviews have highlighted the findings from available research, the gaps in the science, and recommendations for future research. This presentation presents the results of a review of original research on tongue-tie and its treatment via frenotomy. Learn more about the maternal symptoms and signs of problematic feeding that may be associated with tongue-tie in the infant and what the available research tells us about recommendations for infants with tongue-tie.

Bernadette Mazurek Melnyk is Vice President for Health Promotion, University Chief Wellness Officer, Dean and Helene Fuld Health Trust Professor of Evidence-based Practice, the College of Nursing, Professor of Pediatrics and Psychiatry at the College of Medicine and Executive Director of the Helene Fuld Health Trust National Institute for Evidence-based Practice at The Ohio State University. Dr. Melnyk is recognized nationally and globally for her clinical knowledge, expertise in evidence-based practice, mental health, and intervention research as well as her innovative approaches to health and wellness. Dr. Melnyk was the first Chief Wellness Officer appointed at a University in the U.S. She founded and is the current president of the National Consortium for Building Healthy Academic Communities. Dr. Melnyk is an elected member of the National Academy of Medicine, the American Academy of Nursing, the National Academies of Practice, and the American Association of Nurse Practitioners. She is a member of the board of directors for the National Forum for Heart Disease and Stroke Prevention. Dr. Melnyk has over $33 million dollars of sponsored funding from federal agencies and foundations as a PI, is an editor of seven books, and has authored over 450 publications. As a member of the National Academy of Medicine’s Action Collaborative on Clinician Well-being and Resilience, she is working to address the national crisis of healthcare provider burnout.

Abstract:

This presentation will describe the state of translating research and evidence-based interventions into real world clinical settings to improve outcomes. Barriers and facilitators will be discussed. The “so what” outcome as a critical factor in speeding the translation of evidence based interventions into clinical settings will be highlighted. The presentation also will describe two successful programs of research as examples of success in scaling evidence-based interventions into practice. The Advancing Research and Clinical practice through close Collaboration (ARCC) Model will be emphasized as a successful framework for the rapid translation of evidence into practice to improve outcomes.

Amy Holmes has practiced as a pharmacist for 21 years with the last 9 years being focused in neonatal critical care at Novant Health Forsyth Medical Center in Winston Salem, NC. She holds degrees in pharmacy from University of North Carolina (BS) as well as the University of Maryland at Baltimore (PharmD). Amy serves as the Residency Program Director for the acute care PGY1 program at Novant Health Forsyth Medical Center. This presentation will provide a general overview of neonatal abstinence syndrome (NAS) followed by a more detailed discussion of some of the more recently published approaches to treatment. This will include review of alternative dosing strategies for morphine as well as the use of buprenorphine for management of NAS symptoms.

Australia Lisa Amir, MBBS, MMed, PhD, IBCLC

Professor Lisa Amir is a general practitioner and has been an International Board Certified Lactation Consultant since 1989. She is the author of over 120 peer-reviewed articles on breastfeeding. She works in breastfeeding medicine at The Royal Women’s Hospital in Melbourne, Australia. She is a Principal Research Fellow at Judith Lumley Centre, La Trobe University and is the Editor-in-Chief of the open access journal, International Breastfeeding Journal.

Abstract:

Some mothers find it challenging to breastfeed outside the home, and this may contribute to cessation of breastfeeding earlier than planned. To date, breastfeeding women have not been included in most public space designs, such as parks, shopping centres or public buildings. I worked with experts in planning and urban design to explore design features that invite or deter breastfeeding in public. We conducted interviews and focus groups with breastfeeding mothers in three local government areas in Victoria, Australia. We received input from over 80 mothers speaking five languages, as well as health professionals and council planning staff. Many participants reported avoiding breastfeeding in public spaces due to social expectations or physical comfort. Mothers reported that best spaces for breastfeeding were dignified, safe, comfortable, accessible, compatible with their other needs and responsibilities with a high level of amenity. Using these data, we developed design guidelines that outline how a range of everyday shared spaces could become breastfeeding-friendly as well as the optimal design characteristics for dedicated breastfeeding spaces.

Switzerland Johanna Sargeant, BA, BEd, IBCLC

Johanna Sargeant is an International Board Certified Lactation Consultant, teacher and writer based in Zurich, Switzerland. She is passionate about utilising her background in education, biological science, psychology and language to empower parents with empathetic support and evidence-based information through her private practice, Milk and Motherhood.

Originally from Australia, Johanna provides much-needed English-speaking support to many thousands of parents throughout Switzerland and across Europe, and has recently been writing new education modules for the European Society of Paediatric Research and the European Society of Neonatology. She has taught at the University of Zurich, has spoken as a panelist for the WHO's Baby Friendly Hospital Initiative congress in Geneva, has been an expert speaker and facilitator for Google, and has presented at a wide variety of international conferences. The complexities of her personal feeding experiences fuels her passion for providing knowledgeable, guilt-free infant feeding support globally.

Abstract:

Lactation professionals often accompany clients through the process of making difficult decisions: The client who wants to exclusively breast/chestfeed but loves that their partner gives infant formula each evening; The client who wants to and doesn’t want to stop pumping simultaneously; The parents who feel unsure about a potential frenotomy procedure; The client with breastfeeding aversion, desperately struggling with their 18 month old. Lactation professionals aim to provide empathetic care and to give the evidence needed to make informed decisions, and yet there are times where this is not enough and where clients continue to struggle to choose what works best for them. Here, learners will explore some Motivational Interviewing strategies that will actively empower clients, resulting in a significant shift in lactation practice overall. Discover how the strong desire to inform, advise and fix client problems can significantly reduce positive outcomes, and how a focus on the client’s own ‘change talk’ and ‘sustain talk’ can actively mobilize them towards their goal. Learn specific tools to immediately apply in consultations so parents feel deeply supported, feel motivated towards change, feel ownership of their plan, and to ultimately increase the likelihood of their success.
